Scrutiny Questionnaire as part of Self Evaluation
We are pleased to present the revised Self-evaluation questionnaire for completion at the end of your meetings. The questionnaire has been revised in the light of many helpful comments arising from the first quarterly analysis. Could you please ensure that this new version is used for any meetings taking place on/after Monday 24 January 2011. Your contribution and role in completing the questionnare is vital in ensuring that we monitor and evaluate the impact of our work.
